The documents you will be required to submit could include:
1. A completed claim form - obtained from Dignity Group.
2. A certified copy of the death certificate of the deceased.
3. A certifed copy of the deceased’s and the beneficiaries’ ID.
4. A consultant may request for more documents depending on the nature of claim.
